US Navy Says Deadly McCain Collision Was 'Preventable'

The guided-missile destroyer USS John S. McCain arrives pier side at Changi Naval Base, Republic of Singapore on August 21 following a collision with the merchant vessel Alnic MC while underway east of the Straits of Malacca and Singapore.

The ax came down on Navy officers found responsible for the string of crashes that occurred a few months ago, with the Navy firing two more individuals on Tuesday who were on duty the day on the day #USS McCain crashed into an #Oil Tanker off the coast of Singapore.

In the aftermath of the collision, the chief of the US Navy, Admiral John Richardson, said there was no indication the collision was intentional but an examination would include a full analysis of the vessel's cybersecurity. Jessie Sanchez, "exercised poor leadership of the ship's training program", the Navy's Seventh Fleet in Yokosuka, Japan said in a Tuesday statement. Jessie Sanchez, were relieved of their duties on October 11.

The McCain collision is among a series of mishaps, including three collisions and a grounding, that have plagued the Navy this year. The statement also announced that their temporary replacements are former USS McCampbell commanding officer Cmdr.

The Pacific-based US Seventh Fleet confirmed commanding officer Alfredo J Sanchez and executive officer Jessie L Sanchez were "relieved of their duties" on Wednesday. That incident claimed the lives of seven US sailors.

The latest move comes after the US Navy fired the-then commander of the Japan-based Seventh Fleet, Vice Admiral Joseph Aucoin, following the August crash.

The McCain's sister ship, the Fitzgerald, nearly sank off the coast of Japan in June after colliding with a Philippine container ship.

In May, a South Korean fishing vessel collided with the guided-missile cruiser Lake Champlain, while another guided-missile cruiser, Antietam, damaged its propellers in January while anchoring in Tokyo Bay.

The commander of the Navy's Pacific Fleet, Adm. Scott Swift, said last month he would retire early after being notified that he was not in the running to lead the U.S. Pacific Command, which oversees all U.S. military operations in the Pacific.
